Description SHIFT: No Weekends SCHEDULE: Full-time Medical City Dallas RN - Registered Nurse - Cyberknife - Clinical Coordinator FULL TIME POSITION SUMMARY The Cyberknife RN (Registered Nurse) Clinical Coordinator performs direct patient care and act as support person, advocate and educator within the scope of the Registered Nurse Practice Act, with emphasis on nursing process / outcomes for the Radiation Oncology service line. The Cyberknife RN Coordinator: Determine the predictable or unpredictable health status and health needs of patients (individual and family) through interpretation of health data and preventive practice in collaboration with patients and interdisciplinary health care team members. Perform comprehensive nursing assessments regarding the health status of the patient. Formulate a nursing care plan based on determination of nursing diagnoses and identified problems. Develop and implement teaching plans for patients concerning promotion, maintenance and restoration of health, as related to current disease-specific state Provide education, emotional support and age specific care to foster a partnership with the patient and significant others. Provide for the care of multiple patients (individual and family) either through direct care or assignment and/or delegation of care to other members of the healthcare team. Evaluate the patient's (individual and family) responses and outcomes to therapeutic interventions. Provide comprehensive discharge coordination and planning for medically fragile patients with complex discharge needs. Coordinates a plan of care, including appointments, transportation, and education, serving as a liaison throughout the facility and in the community regarding services for this unique population. Follows patients by phone to continue to navigate patient and help coordinate future medical needs. Serves as a supportive point person when patients are readmitted to the hospital. Identifies patterns and issues that arise after discharge and implements solutions that improve the quality of care. Aid in the education process for new and established referral practitioners and outpatient clinics regarding radiation oncology and Cyberknife Medical City is a very \"special\" ANCC Magnet medical center located in far North Dallas that is designed with the health and well-being of the entire family.
